Our Community Team is the first point of contact for families wanting to use Chestnut Tree House and provides a vital link between families and the hospice.
The team consists of specially trained nurses and care support workers who visit all new families in their own homes to introduce them to Chestnut Tree House and inform them of the variety of services they can offer.
All families are allocated a keyworker from the Community Team to discuss any aspect of their childs care and remain a familiar point of contact at Chestnut Tree House.
Other services the Community Team offer include short break care to enable parents to take a much needed break, as well as clinical nursing care, end of life care, emotional support sessions, pre and post bereavement support, and support and training for schools and other professional groups.
The Community Team also run a number of fun activity days, sibling and youth groups to enable children and young people to meet and socialise in a relaxed atmosphere with staff on hand to meet their individual needs.
