Palliative care transition study day

What to expect

The study day will explore:

  • The change in landscape and why young people are living with increased complexity
  • The type of conditions that young people are living with
  • A look at the local data of the young people approaching transition.
  • The burden of care for young people and their carer’s and the help needed
  • The voice of the young person and parent and what worries them
  • The challenges and learning from a paediatric hospice in managing clinical complexity, ventilation, learning disabilities and communication
  • National guidance and recommendations for transition
  • Opportunities for working together

The day will be delivered by a team from Chestnut Tree House, including the Director of Children’s Services, Head of Care, Paediatric Social Worker, Paediatric Counsellor, and Ventilation Nurse Specialist.

Who is the course for?

This study day is aimed at any professionals working in an adult palliative care setting.

With advances in medicine, young people are living longer but with a high level of clinical complexity. These young people will be requiring the support of adult services.
