Community Staff Nurse – West Sussex and Hampshire

Hours: 37.5 hours, p.w. (part-time hours will be considered)
Salary: F.T.E. £30,290- £34,580 depending on depth and variety of experience experience in a similar role

Chestnut Tree House is the children’s hospice for East and West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-shortening conditions.

We are looking to recruit an experienced and enthusiastic staff nurse to provide specialist Children / Young People (CYP) Community palliative care as part of a multi-disciplinary team, working in the Local community.

The post holder will manage a caseload which covers the West Sussex and Hampshire areas to assess and plan care, for CYP with a life shortening illness at home as well as working within an integrated care model providing clinical care alongside the in-house team.

They will be required to work within an on-call rota in order to provide 24hr direct nursing support for end of life care for CYP as required.

This is a challenging yet rewarding role and requires dedication and commitment to ensure children and young people are at the centre of care.
