Community Staff Nurse – West Sussex

Hours

30 – 37.5 hours per week

Salary

£32,620 – £37,230 p.a. (pro-rata for part-time) depending on depth and variety of experience in a similar role

About the role

This is an exciting role for an experienced nurse to provide nursing care in the community setting. The post holder will predominantly work in the community, managing a caseload, providing skilled palliative nursing care to all Children and Young People (CYP) accessing Chestnut Tree House services.

You will be delivering respite care within the home and take the CYP on fun outings and access day care at the hospice.

You’ll have responsibility for key working, supporting both children living with life-limiting conditions and their families. You will be supported with a comprehensive clinical skills training programme and will also be developed and supported to provide end of life care in the home setting.

This role is suited to a dynamic and innovative individual who is committed to service improvement and supporting young adults in transition. The role will cover the area of West Sussex.
