In August 2023 we held an event to launch our new transition pathway. The speakers were filmed for those who were unable to attend, or if you did attend, and want to watch back over the talks.

You’ll find out how members of the Chestnut team can help support you and your young person through your transition journey and hear from a guest parent and young person about some of the challenges they faced along the way.

Most importantly, we want you to share your ideas and let us know what you need so we can make sure that our transition offer works for you and your family. You can do this by: discussing directly with your key worker or social worker or emailing cthadmin@stbh.org.uk
